You may become a Broker with Golden West two ways. You can sign up on line by clicking on How To Become a Broker/Agent or you can call us at 1-800-219-9216 and request an Agent Agreement. You will be asked to sign the Agent Agreement and return it along with a copy of your Life Agent license. |

How many employees must enroll to qualify for a group prepaid dental plan? |

We require a minimum of two non-related employees to qualify for a prepaid dental plan. |

Yes, our prepaid dental and vision plans are available on a voluntary basis. |

How many employees must enroll to qualify for a group PPO plan? |
|
We require a minimum of ten eligible employees and 75% participation to qualify for a group PPO plan. |

Can one of your prepaid plans be sold alongside one of your PPO plans? |
|
Yes, you can sell the prepaid plan with the PPO plan to the same group. We require a minimum of ten eligible employees and at least two employees must enroll in the PPO plan. |
