How do I add an employee to our group dental or vision plan? |

To add an employee to your group dental or vision plan, simply have the employee complete the appropriate enrollment form and then mail or fax the completed enrollment form to Golden West. |

How do I remove an employee from my group dental or vision plan? |
 |
There are two ways to remove an employee from your dental or vision plan. The first option is to complete a Change of Status Form and then mail or fax the form to Golden West. The Change of Status Form can be found in your Administration Kit. The second way to remove an employee is to line out the employee's name on your billing statement and provide us with the termination date when you are processing your monthly bill. Remember, all terminations must be reported within 30 days of the termination date. |

How do I order additional supplies for my employees? |

You can request additional enrollment forms and directories by completing the Supply Request Form and faxing it to Golden West. The Supply Request Form is located in your Administration Kit. You can also request supplies by calling our Marketing Department at (800) 219-9216. |

When will I receive my bill from Golden West? |
 |
You will receive your Monthly Billing Statement by the first of each month. Premium is due on the 15th of the month. |
