How do I change my dental provider? |

It is easy either email us the change, including your name and member number as it appears on your card, or call us at the number listed on your member identification card. All changes made prior to the 15th of any given month will become effective the 1st of the following month. |

Our Member Services department is available Monday through Friday 8 am to 5 pm PST and can be reached at the number listed on your member identification card. |
